a 40 yr old man is 5'8 and 260lbs. who resting heart rate is 85bpm, smokes about a pack a day and drinks 6-8 cups of coffee a day, cholesterol is 190mg/dL and who blood pressure is 135/85, who works 50-60 hours a week. can he start an exercise program without any problems?
He absolutely needs a stress test before starting an exercise program. He has too many risk factors to just jump in. We all hate to hear "now that you're 40", but now that he's 40…… Seriously, he smokes, he's overweight, he drinks too much coffee, his resting bp is borderline-he has to have a stress test.

He needs to start slowly. Walking 30 min a day then gradually moving it up to 1 hour. He should also slowly reduce his caffeine and nicotine which should be easier now that he is exercising more.
Once he is walking at least 1 hour a day, reduced his coffee to two and cigarettes to 5-6 a day, he should start a normal program

That sounds a bit tough. If he's going to start an exercise program, definitely start out slow. Week by week do more.
But exercising won't do a whole lot if he smokes and drinks that much coffee. He needs to cut back on the coffee a bit, and quit smoking all together(we all know that one).

He sure can……. There are many overweight people that have good strong hearts. Some people are overweight and healthy as an OX.. Being overweight or obese doesn't mean your a heart attack waiting to happen. I have seen plenty of skinny people with heart problems. Its genetic.. Some people that smoke can live to be 100.. Everyones body is different and some bodies are strong and can tolerate anything thats thrown at them

At 52 I was over weight. Started doing some push ups in the mornings before I left for work. Had bad heart burn one night and a pain in my left shoulder blade. Dismissed both as poor eating discretion and push ups. All most cost me my life. Check with your doctor first.
